Melanotan II is being investigated for several potential therapeutic applications, though the clinical evidence remains limited and raises important safety concerns.

Vitiligo Treatment

The primary legitimate clinical application under investigation is for vitiligo repigmentation. Currently, there is one active Phase 2 clinical trial (NCT07437560) examining Melanotan II as an adjunct to narrow-band UVB (NB-UVB) phototherapy for stable nonsegmental vitiligo. This study is recruiting participants to evaluate whether the compound can enhance repigmentation when combined with standard UV light therapy. However, results from this trial are not yet available.

Metabolic and Neurological Research

Laboratory studies in animal models suggest potential applications in metabolic disorders and stress-related conditions. Research in male rats demonstrated antidepressant-like and antistress effects in chronic unpredictable stress models, with Melanotan II showing activity similar to other ACTH(4-10) synthetic analogs. Additional preclinical work has examined its role in metabolic signaling pathways, particularly in models of serotonin receptor dysfunction, though these remain early-stage investigations.

Safety Concerns and Adverse Effects

Clinical case reports have documented concerning adverse effects associated with Melanotan II use. Changes in oral mucosa have been reported following injections, and there are case reports suggesting a possible association between nasal spray formulations and oral mucosal malignant melanoma. These safety signals are particularly important given the compound's widespread illicit use for cosmetic tanning purposes.

Current Clinical Status

Despite some research interest, Melanotan II lacks regulatory approval for any clinical indication. The available evidence consists primarily of preclinical studies and case reports, with only one ongoing clinical trial for vitiligo. The safety profile remains poorly characterized, and the documented adverse effects raise significant concerns about its therapeutic risk-benefit ratio.
