You, the healthcare provider or organization using AXIFI services, as identified in your account registration.
By creating an AXIFI account and accepting our Terms of Service, you agree to the terms of this Business Associate Agreement.
What Our BAA Covers
BAA Highlights
Automatic Acceptance
The BAA becomes effective when you create your AXIFI account and agree to our Terms of Service. No separate signing process is required for standard accounts.
Breach Notification
We will notify you of any breach of unsecured PHI within 15 business days of discovery, including all information required under 45 CFR 164.410 for your own notification obligations.
Subcontractor Compliance
All subcontractors who may access PHI are required to sign agreements with substantially similar restrictions and protections as this BAA.
Data Return/Destruction
Upon termination, we will return or destroy all PHI in our possession within 30 days, retaining only what is legally required with continued protections.
Access & Amendment Rights
We support your patients' rights to access and amend their PHI, and provide accounting of disclosures as required by 45 CFR 164.528.

Do I need to sign a separate BAA document?
No. When you create an AXIFI account and accept our Terms of Service, you automatically agree to this Business Associate Agreement. The BAA becomes effective immediately upon account creation.
Can I get a custom BAA for my organization?
Yes. Enterprise customers may request a custom BAA with organization-specific terms. Please contact enterprise@axifi.ai to discuss your requirements.
What security measures does AXIFI implement?
We implement comprehensive security controls including AES-256-GCM encryption at rest, TLS 1.3 encryption in transit, role-based access controls, multi-factor authentication, comprehensive audit logging, and regular security assessments. Visit our Security page for details.
How do I report a potential security incident?
