Known Side Effects:
Alendronate, a bisphosphonate used in osteoporosis management, is generally well-tolerated, but it can have side effects. Common side effects include gastrointestinal issues such as abdominal pain, dyspepsia, constipation, diarrhea, and nausea. Less commonly, patients may experience musculoskeletal pain, headaches, and esophageal irritation.

Serious but Rare Side Effects:
More serious side effects, although rare, include osteonecrosis of the jaw (ONJ) and atypical femoral fractures. There is also a potential risk for esophageal carcinoma, although the evidence is not definitive.

Contraindications:
Alendronate is contraindicated in patients with certain conditions:

  • Abnormalities of the esophagus that delay emptying, such as stricture or achalasia.
  • Inability to stand or sit upright for at least 30 minutes.
  • Hypocalcemia (low blood calcium levels).
  • Known hypersensitivity to any component of the product.

Drug Interactions:
Alendronate may interact with non-steroidal anti-inflammatory drugs (NSAIDs), potentially increasing gastrointestinal side effects. Concurrent use with calcium supplements, antacids, or other oral medications containing multivalent cations may interfere with alendronate absorption if taken simultaneously. Patients should take alendronate at least 30 minutes before any other medication, food, or drinks (other than water).

Populations to Avoid Use:
Alendronate should be used cautiously, if at all, in certain populations, including:

  • Patients with severe renal impairment (creatinine clearance <35 mL/min).
  • Pregnant or breastfeeding women due to limited evidence of safety.
  • Individuals with known hypersensitivity to bisphosphonates.
